Medical Needs

At Queensbury Academy we support students with a range of medical needs and work closely with Health Care professionals and parents to provide the best possible care.

We have an identified first aid room in the Central Building and a number of staff have first aid certificates. Students with more complex medical needs have Individual Health Care Plans which detail the areas of concern and provide care information in more detail. 

It is a parental responsibility to inform the Academy of their child’s medical needs, and we request the Medical Information Form below is used to inform us as appropriate.

Medication can only be administered to students if a Consent to Administer Medicines form is signed (also below) and it would be detrimental to the pupil's health not to administer the medication during the school day.

Please return all forms to the Medical Officer who is based in the first aid room in our Central Building.
